For sensitive or barrier-compromised skin, the honest first answer is often: skip exfoliation entirely until your barrier has recovered. But once skin is more stable, some exfoliation methods are genuinely gentler than others — the key is choosing the right type and a conservative frequency.
Physical scrubs: usually the wrong choice
Gritty physical scrubs create micro-tears in already-compromised skin, which can worsen barrier damage even though they feel like they’re “working.” This is one of the more common ways people accidentally extend a sensitivity flare-up.
Gentler alternatives, ranked by mildness
1. PHA-based exfoliants (mildest)
Polyhydroxy acids like gluconolactone have a larger molecular size than AHAs, so they penetrate more slowly and are generally the best starting point for reactive skin. a PHA toner or serum used once or twice weekly is a reasonable starting frequency.
2. Low-concentration lactic acid
Lactic acid is a humectant in addition to being an exfoliant, which makes it somewhat gentler than glycolic acid for sensitive skin. Concentrations under 10% are a safer starting point.
3. Enzyme exfoliants
Fruit-derived enzymes (papain, bromelain) work on the surface only and tend to be well tolerated, though they’re also less consistently effective than acid-based options.
If your skin is actively red, stinging, or visibly irritated, exfoliation of any kind should wait. Focus on barrier repair first — see signs your skin barrier is damaged — and reintroduce exfoliation slowly once skin has stabilized.
